Medical Authorization

I authorize the veterinarians and staff of Out West Veterinary Center to provide veterinary care, diagnostics, medications, and treatment to my pet should illness, injury, or an emergency arise during boarding.

I understand that every reasonable effort will be made to contact me prior to treatment; however, if I cannot be reached, I authorize the attending veterinarian to provide medical care deemed necessary for my pet's health and well-being.

I understand that I am financially responsible for all veterinary services, diagnostics, medications, treatments, and hospitalization provided during my pet's stay.

Vaccination Requirements

Dogs:

• Rabies — yearly
• DA2PP/Leptospirosis — yearly
• Canine Influenza — yearly
• Bordetella — every 6 months

Cats:

• Rabies — yearly
• FVRCP — yearly
• FeLV — yearly

Pets overdue on required vaccines must receive them at least fourteen (14) days before boarding. Pets vaccinated fewer than 14 days prior to boarding may be denied admission and turned away at check-in.

Vaccination reduces risk but does not guarantee protection from contagious disease.

Contagious Disease Acknowledgment

I understand that despite vaccination requirements, health screenings, cleaning protocols, and preventive measures, boarding facilities carry an inherent risk of exposure to contagious diseases, including Bordetella (Kennel Cough), Canine Influenza, Canine Infectious Respiratory Disease Complex (CIRDC), and other contagious respiratory infections.

If my pet develops signs of a contagious illness during boarding, Out West Veterinary Center reserves the right to require immediate pick-up of my pet.

I agree to retrieve my pet promptly upon notification and understand that all costs associated with examinations, diagnostics, medications, treatments, hospitalization, isolation, and follow-up care are my sole financial responsibility.

Parasite Control Policy

All boarding pets must arrive free of fleas and ticks. If parasites are observed during my pet's stay, I authorize Out West Veterinary Center to administer appropriate treatment, and I accept financial responsibility for all related charges.

Payment Policy

Boarding fees are due in full at the time of pick-up.

Prepayment is required at drop-off for pets boarding longer than fourteen (14) days and for pets scheduled to be discharged on Sundays or other days when payment cannot be collected at pick-up. Additional charges incurred during boarding, including medications, diagnostics, medical treatments, parasite control, emergency care, or other services, will be added to the final invoice and are due at discharge unless prepaid arrangements have been made.

Abandonment Policy

In accordance with Arkansas Code §17-101-313, an animal abandoned in the care of Out West Veterinary Center may be disposed of after written notice is sent to the owner's last known mailing or email address.

The owner will have twelve (12) days from the date notice is issued to reclaim the pet and satisfy all outstanding balances.

Failure to retrieve the pet within twelve (12) days after written notice may result in the pet being considered abandoned and ownership being relinquished to Out West Veterinary Center.
